Case Management/Supportive Counselling & Services - Mental Health

Program Description

Provide support in the community to individuals 16 years plus who experience a serious or persistent mental illness and substance use issues. This may include individual service planning, symptom management, supportive counselling, life skills training, medication support, education, and advocacy. Model of service is recovery. Wait time is for long term case management. Brief service and consultation available as required.
