Case Management/Supportive Counselling & Services - Mental Health

Program Description

Provide support in the community to individuals 65 years of age and older who experience a serious mental illness. This may include individual service planning, symptom management, supportive counselling, life-skills training, medication support, education, and advocacy. Model of service is recovery. CMHA Oxford collaborates with various community partners, including: BSO Teams in the hospital, the Alzheimer's Society of Ontario and the South West LHIN to ensure every is connected to the appropriate services.
