Case Management/Supportive Counselling & Services - Mental Health

Program Description

Clients who are currently in custody can access the service of the Justice Case Manager. The worker will assist the client in identifying their needs post-discharge. The worker will develop a discharge plan in collaboration with Sudbury District Jail personnel, and contact any relevant services to set up initial visits for the client. Once discharged, the client is able to access support and follow-up from this worker for three months. Model of service is psychosocial rehabilitation and recovery.

Access Information

Family members and friends can access the services initially on behalf of the client. If a client is in custody, they should request the service through the Classification Officer who will contact the worker to come in to meet the client.
