Program Description

This program provides psycho-geriatric assessment and consultation; counselling and support to individuals and family/caregivers; home visits; psycho-geriatric clinics at long-term care facilities; advocacy, education and information resource. Program operates in private homes, at long-term care facilities and hospitals in Huron-Perth. Model of service is assessment, counselling and treatment.

Access Information

Client supported while waiting for specialized services. This program is for clients aged 65 and older. Clients between the ages of 60 and 65 are accepted provided they have an age related dementia. Open access. Psychiatrist consultation by physician referral only. Team travels to homes throughout Huron and Perth Counties. Will go outside counties with Huron Perth psychiatrist assignment.
