Program Description

A program to enhance the quality of our older population with complex mental health needs and to support their families and other caregivers in addressing their needs. The team comprised of a Psychiatrist, Registered Nursing and Social Work staff and an Occupational Therapist provides assessment and treatment recommendations in the long term care setting, retirement home or in their home. The team works with care staff and family to support the client with stabilization.

Program referrals must be endorsed by their family physician. To obtain a referral form please contact The Canadian Mental Health Association. All referrals are reviewed by the team and prioritized according to need.
