Case Management/Supportive Counselling & Services - Mental Health

Program Description

Mental Health Services of Renfrew County, Pembroke Regional Hospital provides a case management service that is available to people experiencing a mental illness (schizophrenia, bipolar disorder, clinical depression or other serious mental illness). The illness must be causing an impairment for the person, i.e., not able to perform daily tasks, keep a job, go to school etc. The illness must also be of a long-term nature. The functions provided by the case management program include: intake and assessment, goal setting, assistance with linkages to other services, i.e., legal, financial, medical and psychiatric, housing and other necessary community supports. The caseworkers offer regular support to meet the client's needs. Model of service is recovery.

Access Information

All referrals to the Mental Health, Pembroke Regional Hospital program for all categories of service are directed through a central intake. A physician's referral is not required. Anyone can refer by either self-referral or with the referred person's knowledge and consent. The intake process takes approximately two to three weeks.
