Program Description

Provide assistance to help transition seniors with complex mental health needs from one environment to another supported environment including Long Term Care Home. Social work services are also available to families who have a senior with a mental illness living with them, and who are experiencing difficulties. Provide services across Muskoka & Simcoe County. Average length of stay for social work component is as long as necessary. For transition, clients may stay up to six weeks after placement in long term care facility. Model used is PIECES.

Access Information

Clients must: live in North Muskoka LHIN; if over 65 years of age, be suffering from a serious mental illness and/or neurodegenerative disease; if under 65 years, must suffer from a neurodegenerative disease process such as Alzheimer's Disease; exhibit behavioural disturbances associated with cognitive impairment interfering with ability to function in their environment; be involved with a healthcare professional.
