Assertive Community Treatment Teams

Program Description

The ACT Team is a client-centered, recovery-oriented mental health service delivery model for people aged 18 to 65 with serious and persistent mental illnesses, primarily schizophrenia, psychotic disorders and mood disorders. ACT services are available 24 hours a day, seven days a week. Services are provided on a long-term basis with flexible degrees of intensity. Seventy-five percent of services are provided outside the office base, in homes and local communities. An assertive multi-disciplinary approach with emphasis on engagement, outreach, relationship building and collaboration with support networks. ACT will deliver comprehensive services to individuals in the context of their everyday life and will do this by providing treatment, rehabilitation and support and crisis activities in creative and effective ways.

Access Information

Clients served by ACT are individuals with serious and persistent mental illness or with severe functional impairments, who have not responded well to traditional outpatient mental health care and psychiatric rehabilitation services. Persons served by ACT often have co-existing problems such as homelessness, substance abuse problems, or involvement with the judicial system.
