North Simcoe Assertive Community Treatment Team

Type of Service

Assertive Community Treatment Teams

Program Description

The Assertive Community Treatment Team (ACTT) serves complex clients with serious mental illness who have very significant functional impairment. These clients may have been inadequately served or may have gone without services because of the limitations of traditional mental health services. ACTT services are delivered by a group of multidisciplinary mental health staff who work as a team and provide the majority of treatment, rehabilitation, and support services clients need to achieve their goals. The Act Team has a psychiatrist, three registered nurses, a social worker, an occupational therapist, case managers, peer worker, vocational worker, aboriginal liaison, and an addictions worker. The service is individually tailored for each client and is not time limited. The model of service is client centred and recovery oriented mental health service delivery.
