Assertive Community Treatment Teams

Program Description

This team provides services to persons with serious and persistent mental illness who have complex needs and very significant impairments that have presented as barriers to discharge from Providence Care. The primary objective is to provide individualized support, treatment and rehabilitative services for those clients residing at Providence Care who identified a goal to live in the community. The team provides ACT services to those residing in Kingston, South Frontenac, Lennox and Addington. Model of Service is Recovery, Psychosocial Rehabilitation and Assertive Community Treatment.

COVID-19 Note

Face to face visits occurring using physical distancing and PPE protocols. When feasible meeting outside Pause of family support groups choosing to substitute with 1:1 support Transportation in vehicles resumed with use of PPE

Access Information

Referrals are accepted from family physicians, psychiatrists, and community mental health agencies. When a community mental health worker refers, the referral must be endorsed by the family physician. If it is not clear where to direct a referral, please do not hesitate to call any of the services and the intake clinician will assist you.
