Assertive Community Treatment Teams (ACTT)

Program Description

This team provides services to individuals with serious and persistent mental illness who have complex needs and very significant impairments that have presented as barriers to discharge from Providence Care. The primary objective is to provide individualized support, treatment and rehabilitative services for those individuals residing at Providence Care who identified a goal to live in the community. The team provides ACT services.

Access Information

Referrals are accepted from family physicians, psychiatrists, and community mental health agencies. When a community mental health worker refers, the referral must be endorsed by the family physician. If it is not clear where to direct a referral, call any of the services and the intake clinician will assist you.
