Support Within Housing - Mental Health

Program Description

Support services to persons residing in organization's owned and rent-supplement units, homelessness support services, and transitional housing units including support services to service recipient including 24/7 telephone crisis response, support services, housing support services, advocacy, and life skills.

Access Information

For the supportive housing program referral can be made by the client, family, or health care provider. Referrals for our Support and Housing Programs are processed by one-Link, a team that reviews referrals in our area to make sure that people who need support receive the right services for them. The fax number for one-Link is included on these referral forms. After a referral is made, individuals will be contacted by a one-Link Service Coordinator to talk about the referral in more detail. Client must be medication compliant and have a source of income at time of admission.
