Program Description

Using a strength-based, and client-centered approach, the Youth-in-Transition Worker supports youth in Simcoe County to successfully transition out of the care of the Children’s Aid Society (CAS) and into adulthood. This program supports youth in identifying, developing and implementing a personal plan based on their goals for personal independence.
Delivered through an outreach model, youth can receive the following supports:
• Housing supports
• Education, employment services and training
• Life skills training (financial management, household management, etc.)
• Referrals to health and mental health services
• Access to legal resources
• Pro-social activities
• Safety planning

Access Information

Youth aged 16-24 who are transitioning from CAS care or on a Voluntary Youth Service Agreement (VYSA). Can be referred to the program by a community partner or can be self-referred.
