Program Description

The Peer Support Transitional Discharge Model (TDM) program offers groups and one-to-one services to individuals who are looking for additional support while experiencing an inpatient mental health stay at the hospital and/or following discharge and transition back into the community. Support is offered on a time limited basis.

Access Information

Referrals are made by Woodstock General Hospital staff and the TDM Peer Support Worker who is based at the hospital. TDM support can also be requested by mental health inpatients by speaking with the TDM Peer Support Worker. Referrals for the TDM program are limited to individuals who have been discharged from hospital for mental health and/or addictions related reasons within the past 3 months.
