Program Description

The Peer Support Transitional Discharge Model (TDM) program offers one-to-one services to individuals (Peers) who are looking for additional support following discharge from an inpatient mental health stay. One-to-one support is offered on a time limited basis.

Access Information

Self and professional referrals accepted. Complete online application. Referrals are limited to individuals who have been discharge from hospital for mental health and/or addictions related reasons within the past 3 months.
