Support Within Housing - Mental Health

Program Description

The Back to Home Program is a highly supportive treatment-based community housing program for individuals who have Addiction and Mental Health issues and are occupying Alternate Level of Care beds in hospital or in Long Term Care and who no longer require acute level of care. The Back to Home program provides a safe, supportive, and healthy living environment for eligible individuals.

Access Information

Referrals only accepted for individuals in Alternate Level of Care beds and must come from a hospital, Long Term Care or Mental Health facilities. If hospital staff or Long-term care facility staff (SW, SSW, Nurses, Doctors etc.) identify a person for a referral, contact for more info and to receive a copy of the referral forms. Part of the referral process will be providing various reports related to physical, mental health as well as substance use related concerns.
