Program Description

Coordinated Service Planning offered to families of children and youth with multiple service needs to ensures there is a clearly identified staff for each family to lead their service plan.

Access Information

Anyone can refer a child or youth 0 – 18 (or age 18 – 21 and still attends school) to Contact Brant services, including a child, youth, parent, guardian or professional. Call to access supports. Messages left after hours will be returned the next business day.
