Program Description

Primary worker assigned to each client. Assessment must be completed before case management service. Case management consists of individual and group support, advocacy and referrals to other community-based programs and services.

Access Information

Referrals are accepted from all sources. Self-referrals are preferred. Open to clients treated at other organizations where assessment has been completed.
