Program Description

Community Homes for Opportunity (CHO) is a supportive housing program for individuals with serious mental illness. The program is intended to assist tenants by providing appropriate housing and support services to achieve and maintain stability in a safe, affordable home. The aim is to improve and/or stabilize individual's physical and mental health and foster independence, while enhancing participation and integration into the community.

Access Information

Individual, families and others involved with the individual are encouraged to contact for information and referral. Individuals placed on the wait list will be contacted initially for the purpose of triage
and then regularly for updates. Individuals not accepted for services are provided with information on other possible supports
possible.
