Program Description

A Housing First program for adults living in Toronto who are homeless and are experiencing mental health issues. Intensive case management and rent subsidies are combined to support individuals.

Access Information

Referrals are accepted from anyone, including self-referrals.
Referrals are through The Access Point. Please call or fill out the application form online.
Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.
