Early Psychosis Intervention

Program Description

On Track is a voluntary rehabilitative program that provides care for individuals who are experiencing symptoms of psychosis or early symptoms of a possible psychotic illness. The program provides diagnostic clarifications and recommendations, mental health assessment and treatment, case management, family and patient education, and support. Goals of treatment are remission of symptoms, community re-integration, and recurrence prevention.

Access Information

Referrals are accepted from anyone, including individuals themselves. Call the intake case manager. Care providers can complete the online referral form.

Referral criteria includes: symptoms of psychosis or early psychosis; six months or less of anti-psychotic treatment; reside in the Champlain District.
