The case management program offers supports to adults living with chronic and severe mental health concerns.

Providing assistance with health services, including physical, medical and emotional health and education, everyday living including housing self-care, nutrition and meal preparation, education and training, and family support through education, training, supports, and interaction.

There is an open referral process(self, physician, and community organizations) with home visits available.
