Program Description

Community Treatment Order (CTO) Coordination Services offer the following services:
- develop comprehensive and client-centered community treatment plans in consultation with the key stakeholders (i.e., physicians, clients, families, substitute decision makers, community organizations)
- assist physicians and clinical staff to appropriately issue a CTO
- provide the relevant forms and to be available for consultation regarding challenges that may arise
- provide information and support to clients and advocate on their behalf for services that would meet their identified needs
- facilitate linkages to required community based services and supports for clients leaving a hospital setting
- provide education and information about CTOs and related mental health legislation as requested
- collaborate with the case management service providers to ensure the appropriate services are available and offered to the clients
- develop partnerships and strong working relationships with the various hospitals and community based organizations providing care to our clients
- continually assess the effectiveness of CTOs as a treatment option for individuals with a serious mental illness
- monitor the factors that lead to positive outcomes of CTOs and the challenges in regards to service pressures, resource allocations that may impact on client outcomes

CTO can be renewed if criteria are met and this treatment is required.

Access Information

Anyone can make a referral to the Community Treatment Order (CTO) Coordination Services, however a physician is required to issue and supervise a CTO.

The individual must have a serious mental health diagnosis and have had two admissions to a psychiatric facility in the past three years or one admission that lasted more than 30 days, or the person could have been on a CTO in Ontario to qualify for another CTO.
