Case Management - Mental Health

Program Description

Offers flexible, portable and individualized supports to individuals with complex mental health needs/concerns which seriously affect the individual's life. Promotes independence and quality of life through the coordination of appropriate services and the provision of constant and on-going support, as needed. Case managers assist individuals in setting personal recovery goals and arrange/coordinate the support required to achieve those goals, involving external client support teams where appropriate.

Access Information

Referrals are accepted from anyone, including self-referrals.
Referrals accepted from directly from CAMH and St. Joseph's Family Health Team.

Referrals are also accepted through The Access Point. Please call or fill out the application form online.
Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.
