Case Management - Mental Health

Program Description

Case Management Services provides recovery-oriented treatment, rehabilitation and support for those who experience symptoms of a serious mental illness and who are living in the community. The team assists individuals who are ready to 'step down' from the more extensive supports provided by the ACT teams. The team assists individuals to increase their quality of life and success in community living.

Access Information

Referrals are made from care providers and family members, however, individuals are able to self-refer. There is no restriction to access the program, however the standards set by the Ministry of Health state that for admission there are specific criteria for service. Priority is given to persons with a diagnosis of schizophrenia, bipolar disorder, and other psychotic disorders that have significant functional challenges.
