Case Management - Mental Health

Program Description

Case management services are provided to individuals with mental illness and developmental challenges.

Services are designed to promote recovery and independence while providing appropriate community based support.

Access Information

This program is part of a centralized screening and intake process. Referrals can come from families, self, psychiatrists, and community organizations.

To qualify for the program, the individual must agree to be an active participant in the recovery/ rehabilitation process. The candidate must have a primary diagnosis of a serious mental illness.
