Case Management - Mental Health

Program Description

This program provides case management services to youth that have either a confirmed diagnosis of mental illness or symptoms of mental illness.

Services are designed to promote the skills necessary for youth to live independently in the community while coping with the symptoms of mental illness.

Access Information

This program is part of a centralized assessment and intake process. Referrals can come from families, self, psychiatrists, and community organizations.

To qualify for the program, the individual must agree to be an active participant in the recovery/ rehabilitation process. Individuals must be showing signs of a mental health issue to access this program.
