Case Management - Mental Health

Program Description

Rapid Response System Navigation provides short-term brokerage case management services.

The program is designed to engage, assess and assist individuals in addressing issues related to substance use, mental illness, housing and other stressful life events and to connect to the services and supports that will assist them to fully participate in our community.

Access Information

Referrals are received from the Mental Health Assessment Team (MHAT) of the Thunder Bay Regional Health Sciences Centre, community organizations, psychiatrists, and self-referrals are accepted.
