Case Management - Mental Health

Program Description

The provision of short-term case management for individuals who have a desire to address their serious mental illness and/or substance use. Individuals who have recently been in conflict with the law including, but not limited to, individuals who have been recently incarcerated.

The case management approach will be brief (one day to four months depending on the individual's needs), providing referrals to appropriate community resources including housing, medical and financial while supporting the individual as they engage the mental health and/or addictions systems.

Access Information

All referrals received can range from self to community professionals including but not limited to correctional facilities.
