Case Management - Mental Health

Program Description

Individuals who are currently in custody can access the service of the Justice Case Manager.

The worker will assist with identifying their needs post-discharge. The worker will develop a discharge plan in collaboration with Sudbury District Jail personnel, and contact any relevant services to set up initial visits for the individual. Once discharged, access to supports and follow-up from this worker for three months.

Access Information

Family members and friends can access the services initially on behalf of the individual. If in custody, request can be submitted to the Classification Officer who will contact the worker.
