Case Management - Mental Health
Individual assessment and planning; skills teaching; coordination and collaboration to ensure linkages to needed community resources; assistance to identify, build and maintain natural supports (e.g. family, friends); advocacy to meet basic needs such as housing, income and medical care; social, recreational, vocational programs, and group supports.
Call to make an appointment with the intake worker. An application process will follow, which will assess the support program best suited to meet the individual's needs.
