Case Management - Mental Health

Program Description

Case management for Durham Region residents aged 16 and over who have serious, chronic mental health issues, want the services offered, and are inadequately linked to needed community supports.

Services include; assessment, case planning, advocacy, referral, and skills training. The program will benefit people needing help meeting basic needs, such as housing, finances, education and employment, emotional support, and crisis management.

Access Information

Applications can be made by the individual requesting service or by a third-party referral (family member, psychiatrist, community support, etc.)
