Program Description

Addiction, concurrent disorder and problem gambling assessment, treatment and follow-up. Offers relapse prevention, family intervention, group treatment and support. Main treatment models: psychoeducation and cognitive-behavioural.

Inclusion criteria:
- People accessing this service must be Portuguese language-dependent
- Patients must reside in our catchment area: Parkside Drive to the west, Yonge Street to the east, Lake Ontario to the south and Eglinton Avenue to the north.
- Referrals are accepted for individuals who cannot access English language services due to a language barrier.

Access Information

Referrals are accepted from family doctors via the Community Mental Health referral form. Referrals for Intensive Case Management are accepted directly or through The Access Point.
