Case Management - Mental Health

Program Description

Provides case management services that are available to individuals experiencing a mental illness (schizophrenia, bipolar disorder, clinical depression or other serious mental illness). The illness must be of a long-term nature and causing an impairment for the individual, i.e., not able to perform daily tasks, keep a job, go to school etc. Services provided by the case management program include: intake and assessment, goal setting, assistance with linkages to other services and resources. The case manager offers regular support to meet the individual's needs.

Referrals accepted from all sources. Complete the AccessMHA online referral form. For individuals who do not have internet access, or who prefer to call, please call directly to book an appointment.
