Program Description

Case managers provide individualized, flexible and client-directed support to individuals living with severe and persistent mental illness. The focus is on individual strengths, interests and abilities. Service is provided in the individual's environment, and is portable and flexible, following the individual when they change housing.

Referrals accepted from all sources.
Coordinated access provided through Mental Health Community Support Services (MHCSS) based at the Canadian Mental Health Association Ottawa Branch.
MHCSS maintains the waiting list for intensive case management in Ottawa, referring applicants to several local agencies, including Salus.
