Program Description

Case managers provide individualized, flexible and client-directed support to individuals living with severe and persistent mental illness. The focus is on individual strengths, interests and abilities. Service is provided in the individual's environment, and is portable and flexible, following the individual when they change housing.

Access Information

Referrals only accepted from Ottawa Community Housing tenant staff and Ottawa Salus Community Developers.
Individuals must be living in one of the four designated Ottawa Community Housing buildings.
To be referred to this program people must be at risk of losing their housing due to untreated mental health and/or addictions.
