Assertive Community Treatment Teams (ACTT)

Program Description

Specialized mental health services provided by a team of professionals in the individual's home, community or in the hospital. Model of service is active treatment and psychosocial rehabilitation. For individuals diagnosed with a serious mental illness, who have severe and persistent symptoms and impairments, and who have not benefited from traditional outpatient programs.

Access Information

Referrals only accepted from professionals. Physician or hospital referral preferred. Call the Centralized Access program. A completed referral will include a detailed history of the mental health problems, including hospitalizations related to these problems. Problems must be severe, require an intensive team approach to prevent readmissions to the hospital, to improve the quality of life, psychosocial wellness and promote community integration.
