Program Description

Comprehensive Community Support Team (CCST) provides ongoing and episodic care to clients who have reached their potential and require ongoing social, vocational, housing or medication support or who require episodic support to address crises that arise.

CCST - Outreach Program provides support to clients in their homes to maintain medication compliance and to monitor health status. Support is also available to accompany clients to medical and other appointments.

Access Information

Referrals are processed through a centralized intake. Referrals are accepted from family physicians or community organizations through the AccessPoint Northwest.

To qualify for the program, the candidate must agree to be an active participant in the recovery/ rehabilitation process.
