Case Management - Mental Health

Program Description

Justice Program supports individuals who have had recent or long-term involvement in the criminal justice system, and provides coordination of appropriate services and the provision of ongoing support as needed. A broad range of services are provided and tailored to the individual’s needs, including but not limited to:
- Assertive outreach
- Assessment
- Information and education
- Support, coordinate services and links to resources that an individual may need
- Goal Plan based on individual needs
- Supportive counseling
- Crisis-planning, crisis intervention and crisis prevention
- Assistance with symptom and behavior management
- Medication support
- Skill-development
- Family support and education
- Advocacy

Access Information

Referrals come from a variety of sources including lawyers, doctors, hospitals, family members, police, other community agencies and individuals themselves. Call, fax, or walk in at the Crisis & Mental Wellness Centre to speak with a Coordinated Access worker.
