Case Management - Mental Health

Program Description

Focused Recovery Program supports individuals with a serious mental illness through the coordination of relevant services and the provision of ongoing support as needed.

A broad range of services are provided and tailored to the individual's needs, including but not limited to:
- Assessment
- Information and education
- Support and connections to community resources
- Goal planning
- Supportive counselling
- Crisis-planning, crisis intervention and crisis prevention
- Assistance with symptom and behavior management
- Medication support
- Skill-development
- Family support and education
- Advocacy

Access Information

Referrals accepted from all sources - self, family, service providers. Call, fax, or walk in at the Crisis & Mental Wellness Centre to speak with a Coordinated Access worker.
