Program Description

The Housing Outreach Program exists to help chronically homeless individuals living with a severe and persistent mental illness find housing in the community and work towards addressing issues that may have contributed to their homelessness and ultimately work towards their recovery. Case management services provided anywhere in the community where the individual may request support.

Access Information

All referrals to this program come form the city of Ottawa.
Shelter referrals are identified by community homeless shelters and given to City of Ottawa for prioritization and referral to CMHA.
