Program Description

Prevention Diversion serves as the initial point of contact for youth aged 16-24 who are homeless or are at immediate risk of becoming so within the City of Kingston. In addition to preventing and diverting youth from homelessness and accessing shelters, case managers will collaborate with the youth to determine their housing needs and explore realistic housing options. Case Managers also assist with obtaining ID, securing a source of income, establishing connections with mental health and addiction services and any other required support within the community to meet the identified needs of the individual.

Access Information

Youth can attend in person or call to complete a referral form to determine eligibility for services offered within the program.
