Support Within Housing - Mental Health

Program Description

A low support, co-ed group home in North Toronto with a capacity of six people. A residential counsellor is available two evenings per week, with staff on call for emergencies 24 hours a day. Residents must be motivated toward peer support and want to live in a group setting. Residents have their own bedrooms. Model of service is client-directed and recovery using psychosocial rehabilitation principles. Average length of stay is twelve years.

Access Information

Professional referral to Support Within Housing program is preferred but not mandatory. Applications for Bayview Cooperative are processed through The Access Point which is responsible for managing the housing waitlist. Referral forms can be requested by calling Bayview Community Services office, accessing them on Bayview Community Services website, or through The Access Point. Admission criteria: adults 16 years of age and over who have a serious psychiatric disability, no recent street drug or alcohol abuse within the past 6 months, financial status and professional follow-up established prior to admission, and no conviction under the criminal code. They also must manage and monitor own medication, need a long-term supportive environment in order to reach and maintain optimum level of functioning, be motivated to live in a cooperative setting and be involved with daily activity. They must follow non-negotiable house rules.
