Intensive case management services coupled with a housing supplement. These housing units are intended for those with complex addictions, often with concurrent mental health and addictions issues. Eligibility requirements: - homelessness and/or street-involved within the City of Toronto - significant mental and/or physical health issues and referring organization must have current knowledge of client's location. - clients must also meet Addictions Supportive Housing criteria - potential clients would have current or historical problematic substance use, are homeless or at risk of losing their housing, qualify for a housing subsidy, or have a history of high-intensity service use (ER, hospital inpatient, WMS).

A community-based mental health program for women with mental health issues or concurrent disorder who have been homeless for 6+ months. Outreach is utilized to engage the woman, and the workers meet the clients in the community. Works with the clients to meet basic needs, including food, shelter, ID, income, healthcare, and linking to other supports and services.

Assists individuals age 16 and over who are having significant difficulty in day-to-day living as a result of severe and persistent mental health issues. Assists clients to meet their basic needs such as food, housing, clothing, health care, crisis management, identify and meet goals, and connect to resources and services.

A rapid access, intensive, short term geriatric case management program that supports senior adults 55 and over in St. James Town, Moss Park and Regent Park, whose complex mental health illnesses or other issues (e.g. poverty, social isolation, deterioration of physical health, risk of eviction, hoarding and infestation) are impacting their ability to safely maintain their wellness and housing. Provides outreach, assessment and referrals and community based, one to one, short term intensive case management services for those seniors in the identified neighbourhoods, who experience complex mental and physical health care needs.

Health Promotion staff work to support individuals, groups and organizations in the planning, delivery, and evaluation of mental health services in Toronto. This program offers a 16-week structured Cognitive Behavioural Therapy program for individuals with auditory hallucination.

A community-based program that works toward supporting individuals with mental illness who are involved or at risk of being involved in the criminal justice system. The goal of the program is to work with individuals in the community so that they can live successfully and reduce and/or prevent involvement with the criminal justice system. Clients must have active involvement, or risk of involvement, in the criminal justice system.

Supports individuals' transition from custody through probation/parole and the courts to the mental health community. Recognizes that some individuals who face criminal charges would be better served by the mental health system than the criminal justice system. Individuals are diverted out of the regular stream of the criminal justice system by virtue of their voluntary participation in the program.

The purpose of the Mental Health Court Support Program is to assist people charged with low-risk criminal offences to access and utilize mental health services and other community based supports. The Mental Health Court Support Program recognizes that some individuals who face criminal charges would be better served by the Mental Health System rather than the Criminal Justice System (CJS). Individuals are diverted out of the regular stream of the CJS, by virtue of their voluntary participation in the program. The model of services is recovery focused. The average length of stay is 6 to 12 months.

Provides support to individuals in the community who are charged with a low-risk offence. Assists individuals to transition from the criminal justice system and into the mental health system where their mental health, addictions and other needs may be better addressed. The objective is to reduce further criminal charges.

Provides culturally appropriate case management for people experiencing their first episode of psychosis and the people who care about them.

Assists individuals age 16 and over who are having a lot of difficulty in day-to-day living as a result of severe and persistent mental health issues. Case managers assist clients with employment, social advocacy and crisis management.
