Short-term intensive case management services for individuals with mental health and addictions issues who are being released from correctional custody. This includes linking clients to longer-term case management services, rehabilitative programs/medical treatment, safe beds, court support services, and accessing appropriate housing and financial assistance. The program also provides support and information to family members and/or significant others.

Intensive case management for individuals who have a persistent and serious mental illness.

This program works with individuals who are dealing with their first experiences with a psychotic illness and help them to identify their concerns and goals, and to develop plans that work on recovering from psychosis.

Supportive housing for clients who are Six Nations band members and are actively involved with mental health services.

The program provides secure, safe housing for individuals who are Six Nations Band Members, are homeless or at risk of homelessness, and have a complex addiction issue or concurrent disorder.

This service provides outreach and counselling to individuals, families and couples, including addictions treatment groups and therapeutic groups.

Access to intensive, non-residential addictions/substance use treatment services for youth and or adults in Six Nations of the Grand River. The intensive/concurrent Addictions Case Managers will support the day/evening care programming.

Intake worker for Six Nations Mental Wellness providing support for individuals (including but not limited to clients of the RAAM clinic) and helping to receive streamlined access to Six Nations Continuum of Mental Health and Addictions Services. Services can include referrals to others services as appropriate.
