Mental health case management services to individuals requiring follow up from a crisis, or long-term assistance with a goal of reaching an optimum level of functioning in the community. Services include: supportive counselling; links to resources and support networks; advocacy; ensuring goals are met; medication monitoring and health education; administration of medication within a clinical setting; rehabilitation skills and support; enrichment; encouraging fulfilling and satisfying activities; support with budgeting for food and housing.
