The transitional home, Victoria Street Place, will provide individuals who would otherwise have been homeless or at risk, to have decent affordable housing for up to eight months. Transitional housing is intended to offer a supportive living environment and tools and opportunities for social and skills development. It is an intermediate step between emergency shelter and having no shelter to supportive housing, with limits on the length of stay. The program requires cooperative living where the tenants, in conjunction with staff, will share the responsibility of managing the household.

The Healing With Hope - Harm Reduction Home is a residential program that provides managed alcohol services to individuals that are homeless or risk of homelessness that are alcohol dependent. The program provides alcohol dispensing. The program offers access to primary care through program physicians & nurses. Individuals receive assistance to enhance daily living skills, maintain their wellness and support through case management, programming and harm reduction. The program provides advocacy, one to one individual planning (Care Plans), peer support, development of daily living skills, increased problem solving skills, and accessing community resources.

The Housing Case Management program will provide short term support to individuals, linking them to community supports; maintaining their housing, reducing the risk of homelessness and jail; and increasing diversion from hospital. Short-term support where individuals would receive assistance to enhance daily living skills, maintain their wellness and living environments. Short-term support activities would include: crisis prevention, hospital diversion, advocacy, one to one individual planning (Community Support Plans), peer support, development of daily living skills, increased problem solving skills, and accessing community resources.

This program offers support to people with a mental illness/addiction/developmental delay, who are in conflict with the law. The following are services offered within this program: diversion, psychosocial assessments, fitness screenings, support, information, consultation and education.

This program offers a wide range of educational, health promotion, and community development services. Information is provided on mental health and mental illness through promotion, presentations, mental health first-aid workshops, information fairs / events, conferences, workshops, newspaper articles, radio advertisements and our website. Training in Mental Health First Aid, Safe-Talk, Mental Health Works, and Talking About Mental Illness.

Individuals who are currently in custody can access the service of the Justice Case Manager. The worker will assist with identifying their needs post-discharge. The worker will develop a discharge plan in collaboration with Sudbury District Jail personnel, and contact any relevant services to set up initial visits for the individual. Once discharged, access to supports and follow-up from this worker for three months.

The Youth Mental Health Court program supports transitional youth, who have a mental illness, addiction or developmental disorder and have been involved in the Criminal Justice System. The Youth Mental Health Worker will provide consultations to the Crown Attorney, Defence Counsel and other court personnel to link these youth with appropriate mental health services in the community. Youth Mental Health Diversion agreements will be implemented with consent of the youth and his/ her guardian and approval from the Crown Attorney. In order for the youth to be considered an appropriate candidate for mental health diversion, symptoms of mental illness must have been a factor in the offense that was committed.

Clinical Services provides service to individuals from a psychiatric rehabilitation approach. Individuals will look at their readiness for change and set overall rehabilitation goals in the environments of living, learning, working or social. Completion of an OCAN assessment and readiness assessments and if they are not ready to set a goal, they will work with the Practitioner to develop their readiness through one-on-one sessions or group work. Individuals will identify the skills or supports needed through the Functional and Resource Assessment which will assist them in performing the skills at the required level for the goal. Case Managers offer personal support and advocacy. The service of the program is designed to be transitional. Individuals come to the program to get the service they need in order to set goals and regain a meaningful role in their community.

Community Homes for Opportunity (CHO) provides tenants who have a serious mental illness with a recovery oriented supportive housing environment. It offers 24/7 support services to maintain housing stability. There is one 12-bed CHO home in the Greater Sudbury area supporting residents.
