Program Description

Intensive Case Management is employed when the patient is able to sustain their self in the community with routine and weekly support and monitoring of treatment compliance, signs and symptoms. This is not an Assertive Community Treatment Team (ACTT). Model of service is psychosocial rehabilitation. Shared care.

COVID-19: We are providing telephone counselling to current clients. Our psychiatrists are providing telephone, OTN and limited in person appts.

Access Information

Patients access this program as the clinic treatment adjunct through the Adult/Geriatric Mental Health Program. This is a community-based service. A patient must be informed and voluntary. A family physician referral is preferred. Personal information or referrals sent by email or internet applications cannot be accepted or acknowledged. Note: We do not have resources, expertise or security to conduct forensic assessments.
