Case Management/Supportive Counselling & Services - Mental Health

Program Description

We provide a range of clinical services to adults aged 18+. Services are tailored to meet the needs of the individual to provide relevant education, aid in skill development and promote independence. Our case management program provides assessment and individualized psychosocial rehabilitation for individuals with mental illness, and transitional support for clients being discharged from CKHA inpatient programs, as well as waitlist monitoring for elective admissions and ECT. The service is client centered, structured, goal focused and time limited. 24-hour crisis services are also available. Satellite locations are available in Wallaceburg and Walpole Island.

Access Information

Self-referrals or by family, physicians, hospitals or community organizations. All referrals are vetted through the Mental Health First Response Team. Contact CMHA for assistance in making referral. Services are for individuals that have a severe and persistent mental illness causing disability. If deemed appropriate for CMHA services the intake worker will connect within ten days.
