Program Description

Sunnybrook Program of Assertive Community Treatment (SunPACT) is a multidisciplinary team-based program providing community-based, client-centered individualized treatment, support, and rehabilitation to persons with severe and persistent mental illness. Model of service is assertive community treatment. There is no set time limit for staying in the program.

Access Information

Applicants to Sunnybrook Program of Assertive Community Treatment (SunPACT) need to have been diagnosed with a major mental illness and have had 50 days or more of hospitalization days in the past two years. Substantial use of hospital and community mental health crisis or emergency services would also be taken into consideration. Referrals can be initiated by consumer/survivors themselves, family members, and community and health professionals. Referrals are coordinated through The Access Point.
