Program Description

St. Jude Community Homes provides quality supportive housing to single individuals living in Toronto area living with severe mental illness. The organization follows a community development model of housing that promotes individual as well as community development in order to promote recovery and improve quality of life. Model of service is psychosocial rehabilitation and recovery model.

Access Information

Residents must have a Formal Diagnosis of serious mental illness, be on social assistance, manage own medications and be able to live somewhat independently or have the desire to acquire independent living skills. They must also have the desire to live in a "community model" of supportive housing. Exclusions are, drug and alcohol dependency and an active legal or criminal involvement. Access is through The Access Point by community mental health professional, completion of an application and an interview with management staff. The cost associated with this program is rent geared to income and 250.00 per month for a partial meal program which consists of breakfast and an evening meal Monday through Friday.

There are three management and two support staff for days Monday through Friday. There are two support staff for the evening hours 1300-2100 shared with the Dundas East Street site. Staff available on-call after hours.
